This request is for approval of a record plat of subdivision for Autumn Leaves Assisted Living located west of the southwest corner of 151st Street and 80th Avenue.
Staff has reviewed the Record Plat of Subdivision and finds it to be substantially in conformance with the approved Final Plan.
This is now before the Village Board for consideration and to authorize signature of the plat.

I move to approve the Record Plat of Subdivision for Autumn Leaves prepared by R.E. Allen and Associated, LTD., dated 02-19-08, last revised 06-27-08.
